Diagnostic Wax-up Problem Statement »
Inadequate fabrication/design of a diagnostic wax-up!
Purpose »
The purpose of this this evaluation is to emphasize on the lack of appropriate case planning that subsequently
may result in a poor final smile makeover.
Patient's Main Concerns »
Reverse smile line;
Centrals not prominent enough;
Wants "central dominance" and laterals shorter than centrals; |
Canines point wrong direction; |
Dark buccal corridors; |
Doesn't like appearance of crowns on implant teeth; |
Teeth too square - wants minimal reduction!!!! |
Doctor's Prescription » Regarding the existing wax-up:
I have altered #1.2 slightly,
rounding the M-inc corner. The patient likes it better than #2.2 which
hasn't been altered; |
#2.1 looks like it has too much mesial axial inclination. This may be partly due to the fact that the gingiva needs to be contoured slightly (raised) on the mesial;
Centrals have been waxed up as crowns, but wants MINIMAL reduction and should have been done as veneers; |
Patient would like the centrals even more dominant ie. still a little wider, but proportionately so. Are we limited by the width of the adjacent implant abutments? |
The stick-bite does not look horizontal in the photo (not parallel to the inter-pupillary line) and the vertical portion does not look completely vertical either. Horizontally,
it drops down on the patient's left. |
Before - Other Laboratory Work » In this case, the existing wax up does not address any of the patient’s chief concerns and has to be redone in order to create the appropriate blue print for the final restorations.
After - FX Veneers Dental Laboratory Work » Before starting any diagnostic wax-up, the appropriate smile design protocol should be followed and factors listed below should be taken in to the consideration.
Will we be able to achieve?
proper function; |
proper incisal edge harmony with the smile line; |
proper gingival contour shape and position; |
proper lip line; |
proper midline; |
proper axial inclination; |
proper incisal embrasures; |
proper central dominance and golden proportion; |
proper emergence profile; |
optimal periodontal health; |
properly developed buccal corridor ; |
proper cervical embrasures ; |
Sometimes, a perfect final result cannot be achieved due to function and other restrictions that are taken into the consideration prior to the cosmetic concerns. But an exceptional smile makeover can be achieved if planned properly.
Conclusion » In this case, the only unsolved concern is the dark buccal corridor which is caused by the low posterior bone and tissue formation that gives the illusion of darkness and could be corrected by a more extensive treatment.
The gingival contour shape and position could be further addressed in
the subsequent treatment.
